WAIVER OF LIABILITY AND INFORMED CONSENT RELEASE

In consideration of your acceptance of myself as a participant in the services (the “Services”) conducted by Body n’Sync, I hereby agree to the following:

1. I am not less than 21 years of age.

2. I am in good physical condition, and have kept and will keep Body n’Sync informed of all existing medical problems and/or physical injuries or ailments that may be aggravated as a result of my participation in the Services or which may render me unsuitable to participate in the Services or be more susceptible to injury.

3. I understand that I may sustain physical injury or may aggravate any existing conditions because of my participation in the Services and I accept this risk and hereby waive and release Body n’Sync, its employees, agents, and subcontractors from all actions, proceedings, claims, demands, and liabilities related to any such injury sustained or aggravation of any pre-existing conditions during or after my participation in the Services.

4. I am fully aware, and acknowledge, that my participation in the Services may involve physical contact and hereby waive any claim or complaint relating to such contact.

5. I further waive and release Body n’Sync, its employees, agents, and sub-contractors from all actions, proceedings, claims, demands and liabilities in respect to any loss or damage to myself or my personal belongings, sustained or incurred by myself within the premises at which the Services are conducted.

6. I acknowledge that Body n’Sync requires a minimum notification period of 12 business hours for all cancellations. I understand that cancellations less than that or no-shows are subject to a penalty fee

7. I acknowledge that all purchases at Body n’Sync, including packages, goods, and services, are final and nonrefundable. Body n’Sync reserves the right to introduce and vary the prices and categories of services and packages at any time. Body n’Sync packages are valid for 1 year from the date of purchase unless otherwise stated. Expired packages are considered void and contain null value.

8. I acknowledge that Body n’Sync premises are under monitoring by CCTV and consent to Body n’Sync recording me for studio security purposes. I understand that surveillance footage will only be used for monitoring purposes and security-related matters, and footage will not be shared to third-parties without my knowledge and consent.

9. By submitting this form, I hereby verify that I have read all the terms and conditions and that, to the best of my knowledge, all the information I have provided is correct and accurate.
